Facility

Our office is conveniently located off I-84, in Nampa at 16150 N High Desert St. Ste. 112, near the Ford Idaho Center. We offer onsite Massage and Digital X-Rays. Same day appointments are generally available. Most insurances are accepted.

Financial

Our practice accepts cash, check, and major credit cards. We will be more than happy to submit your insurance forms for you, and we accept most major insurance providers. Please call our office to verify whether your specific insurance plan is accepted. Payment plans can be discussed upon request. Additionally, we do accept Care Credit and FHA/FSA cards.

Cancellation

If you would like to cancel an appointment, we ask that you provide us with at least 24 hours notice. We have a busy schedule and respect our patients' time. We do our best to stay on schedule. Missed, or late appointments can affect the doctors time with other patients. A fee may be charged for a no­-show or late cancellation.
